Compensation & Schedule:
- $22.00 – $25.00 per hour (based on experience)
- Performance-based bonuses (including census incentives)

Key Responsibilities:
- Administer medications and treatments per physician orders
- Monitor residents and report changes in condition
- Complete accurate documentation and incident reports
- Follow up on physician orders, labs, and care plans
- Assist with admissions and service plan implementation
- Supervise and support caregiver staff
- Maintain compliance with WV assisted living regulations
- Follow infection control and safety protocols
- Communicate effectively with residents, families, and team
Qualifications:
- Active LPN license in West Virginia (required)
- Graduate of an accredited nursing program
- Experience in assisted living or long-term care preferred
- Ability to work independently and make sound clinical decisions
